Please use this form to switch your membership between the Unlimited (Full) Membership and the Two/Week (either way), however:

You must give a minimum of seven (7) days written notice (in advance), prior to your payment processing day, of your intention to switch (see example below);

Example:

If you want to switch your membership in December, we require your form by the 23rd November as there are 30 days in November and we require 7 days notice.

This allows us seven days to action your request and confirm back via email.

CUT OFF DATES THAT WE REQUIRE THE FORM IN BY - PLEASE CHECK BEFORE SENDING AS WE CANNOT PROCESS IF LATE:

January Switch - please have the form sent in to us no later than 24th December

February Switch - please have the form sent in to us no later than 24th January

March Switch - please have the form sent in to us no later than 20th February

April Switch - please have the form sent in to us no later than 24th March

May Switch - please have the form sent in to us no later than 23rd April

June Switch - please have the form sent in to us no later than 24th May

July Switch - please have the form sent in to us no later than 23rd June

August Switch - please have the form sent in to us no later than 24th July

September Switch - please have the form sent in to us no later than 24th August

October Switch - please have the form sent in to us no later than 23rd September

November Switch - please have the form sent in to us no later than 24th October

December Switch - please have the form sent in to us no later than 23rd November
